Wellness Nurse
The Wellness Nurse is responsible for assisting the Resident Care Director with the overall implementation,delivery and co-ordination of Resident care services in the Community. Nurse will also assist in monitoring themedication management program. The following essential functions are the fundamental job duties of the position to be completed with orwithout appropriate reasonable accommodation.Assist in supervising the medication program and assisting the residents with self-administration of their medicationAssist residents with Activities of Daily Living and Independent with Activities of Daily Living as neededMaintain positive relations with residents, families and physiciansReport any significant incidents and/or changes in the residents needs to the Executive Director/ Resident Care DirectorAssist in the completion of resident assessments and services plansAssist in the training of assisted living staff and participates in orientation of new staffAttend and participates in scheduled in-service programs, training programs and staff meetingsAssist with scheduling and staffing of Resident Care AssistantsManage the Quality Assurance forms assigned to WellnessSupervises All Resident Care Aides. May perform other duties as assignedServes a member of clinical and treatment team providing input on resident goals, interactions and problem solvingInitiates and leads one to one and group interactions that promote confidence, individuality and self-esteemCollaborate and coordinate implementation of behavioral plans with resident aides RN/LPN/Licensed Vocational Nurse with a current license in good standing.
